What is on the sheet
The fifteen conditions of the System Reference Document 5.1, in its order: blinded, charmed, deafened, exhaustion, frightened, grappled, incapacitated, invisible, paralyzed, petrified, poisoned, prone, restrained, stunned and unconscious. Exhaustion has its table of six levels.
The text is quoted as the SRD 5.1 prints it. Nothing is summed up or reworded, so the sheet can settle a ruling at the table. It works with any fifth-edition-compatible game that uses the SRD 5.1 conditions.
